RSI Prevention for Programmers: What the Research Actually Says

Repetitive strain injury is preventable, but the gadgets people buy first are rarely the interventions with the best evidence.
What the clinical evidence actually supports
Repetitive strain injury (RSI) is an umbrella term for micro-trauma to tendons, nerves, and soft tissue caused by high repetition, sustained static muscle load, and poor joint angles.
The single strongest predictor of RSI development is continuous time-in-position without rest. When muscles sustain low-level static contractions for hours, intramuscular pressure rises, restricting blood flow and tissue recovery.
The clinical evidence for scheduled micro-pauses (30 seconds every 20–30 minutes) and neutral joint alignment reducing RSI incidence is strong. The evidence for daytime wrist braces preventing injury is thin.
Preventing RSI is an operational pacing problem, not a hardware problem.
What actually works
Sort your interventions by evidence strength and implement these specific protocols:
- Enforce Micro-Pauses: Use a timer to take a 30-second break every 25 minutes. Drop your hands into your lap and relax your forearms completely. This allows tissue re-perfusion.
- Maintain Wrist Neutrality: Keep your wrists straight (0 to 15 degrees extension max) relative to your forearms. Avoid resting your wrist heels on desk surfaces while typing, as this increases carpal tunnel pressure.
- Avoid Daytime Braces: Do not wear rigid wrist braces while typing. Braces restrict natural movement, forcing surrounding muscles to overcompensate. Reserve splinting for night use under clinical guidance if carpal tunnel is diagnosed.
- Load Triage: If you experience early symptoms (burning, tightness, tingling), reduce typing volume immediately. Use voice coding tools, dictation, or alternative input devices to allow tendon recovery before pain becomes chronic.
Hardware changes like split keyboards help neutralize wrist angles, but hardware alone cannot overcome 10 hours of uninterrupted typing.
